Preparing for Your Visit

Bring a list of all your medications, including eyedrops, or bring the medications in if you are unsure

Know your past medical history, including any past injuries, surgeries, or complications

Arrange for someone else to drive you after the visit as we may use dilating drops that will impact your vision

Please fill out and bring the patient questionnaire to your appointment. Copies of the form will also be available at the clinic.
Please allow at least 2 hours for your initial appointment.
